PENGARUH EDUKASI PERAWATAN KOLOSTOMI TERHADAP PENGETAHUAN DAN KETERAMPILAN IBU DALAM MERAWAT STOMA BAYI DENGAN ATRESIA ANI DI RS X

Abstrak

Atresia ani merupakan kelainan kongenital yang memerlukan tindakan kolostomi sebagai penatalaksanaan awal sehingga keberhasilan perawatan stoma setelah pasien pulang sangat bergantung pada kemampuan ibu sebagai caregiver dalam melakukan perawatan kolostomi secara mandiri, sedangkan masih banyak ibu yang memiliki pengetahuan dan keterampilan yang belum memadai sehingga berisiko menimbulkan komplikasi seperti iritasi kulit peristoma, kebocoran kantong kolostomi, infeksi, hingga meningkatnya angka rehospitalisasi. Penelitian ini bertujuan menganalisis pengaruh edukasi perawatan kolostomi terhadap peningkatan pengetahuan dan keterampilan ibu dalam merawat stoma pada bayi dengan diagnosis post operasi kolostomi di ruang Neonatal Intensive Care Unit (NICU) RS X. Penelitian menggunakan desain studi kasus dengan pendekatan one group pretest–posttest pada satu orang ibu sebagai caregiver utama bayi post operasi kolostomi akibat atresia ani. Intervensi berupa edukasi perawatan kolostomi dilaksanakan selama dua hari berturut-turut, masing-masing satu sesi berdurasi sekitar 45 menit menggunakan metode ceramah, diskusi, demonstrasi, return demonstration, leaflet edukasi, serta evaluasi melalui kuesioner pengetahuan dan lembar observasi keterampilan. Hasil evaluasi menunjukkan adanya peningkatan pengetahuan ibu dari kategori cukup sebelum edukasi menjadi kategori baik setelah edukasi, disertai peningkatan keterampilan praktik perawatan stoma hingga seluruh prosedur dapat dilakukan secara mandiri, benar, dan sesuai standar. Edukasi yang diberikan secara terstruktur terbukti meningkatkan pemahaman, kepercayaan diri, dan kesiapan ibu dalam melakukan perawatan kolostomi di rumah sebagai bagian dari discharge planning. Dengan demikian, edukasi perawatan kolostomi direkomendasikan sebagai intervensi keperawatan yang terintegrasi dalam pelayanan NICU untuk meningkatkan kompetensi caregiver, mencegah komplikasi stoma, serta mendukung keberlangsungan perawatan bayi secara optimal di rumah.

Anal atresia is a congenital anomaly requiring a colostomy as initial management; consequently, the success of post-discharge stoma care relies heavily on the mother's ability—as the primary caregiver—to independently manage the colostomy. However, many mothers lack adequate knowledge and skills, creating a risk of complications such as peristomal skin irritation, colostomy bag leakage, infection, and increased hospital readmission rates. This study aimed to analyze the impact of colostomy care education on improving the knowledge and skills of mothers caring for the stomas of infants who had undergone colostomy surgery in the Neonatal Intensive Care Unit (NICU) of Hospital X. A case study design with a one-group pretest-posttest approach was employed, focusing on a mother acting as the primary caregiver for an infant who had undergone colostomy surgery due to anal atresia. The intervention consisted of colostomy care education delivered over two consecutive days—with one 45-minute session per day—utilizing lectures, discussions, demonstrations, return demonstrations, and educational leaflets, alongside evaluations via a knowledge questionnaire and a skills observation checklist. Evaluation results showed an improvement in the mother's knowledge from the "adequate" category prior to education to the "good" category afterward, accompanied by enhanced stoma care skills, enabling her to perform the entire procedure independently, correctly, and in accordance with standards. Structured education proved effective in increasing the mother's understanding, confidence, and readiness to perform colostomy care at home as part of discharge planning. Therefore, colostomy care education is recommended as an integrated nursing intervention within NICU services to enhance caregiver competence, prevent stoma-related complications, and support optimal ongoing care for the infant at home
