Abstrak

Jaminan Kesehatan Nasional (JKN) merupakan instrumen utama perlindungan finansial kesehatan di Indonesia. Keberlanjutan program tidak hanya ditentukan oleh luasnya cakupan kepesertaan, tetapi juga oleh kemampuan sistem menjaga keseimbangan pembiayaan, mengendalikan biaya, memastikan ketepatan sasaran subsidi, mengoptimalkan mekanisme pembayaran provider, dan mencegah fraud serta moral hazard. Artikel ini bertujuan menyintesiskan materi kelompok mengenai struktur dan tantangan implementasi JKN, utilitas asuransi kesehatan, pembayaran provider, digitalisasi, analisis kebijakan, dan strategi ketahanan pembiayaan. Metode yang digunakan adalah kajian naratif-deskriptif terhadap materi perkuliahan, regulasi, publikasi kelembagaan, teori ekonomi kesehatan, dan pendapat ahli yang telah dihimpun dalam naskah kelompok. Hasil kajian menunjukkan bahwa tekanan pembiayaan JKN berkaitan dengan peningkatan pemanfaatan layanan, penyakit berbiaya tinggi, kepatuhan iuran, ketepatan sasaran PBI, ketimpangan fasilitas kesehatan, serta risiko fraud. Perspektif Hasbullah Thabrany, Nailul Huda, Timbul Siregar, dan Febriansyah Budi Pratama memperkuat pentingnya disiplin iuran, transparansi data, pengendalian moral hazard, penegakan anti-fraud, perlindungan finansial, dan penguatan pelayanan primer. Ketahanan pembiayaan perlu didukung strategic purchasing, pembayaran provider yang berorientasi mutu dan efisiensi, Health Technology Assessment, digitalisasi, penguatan promotif-preventif, integrasi data, serta tata kelola yang akuntabel. Dengan strategi tersebut, sustainability JKN dapat dijaga tanpa mengurangi prinsip gotong royong, keadilan, dan akses terhadap pelayanan kesehatan yang dibutuhkan.

The National Health Insurance (JKN) program is the primary instrument for health financial protection in Indonesia. The program's sustainability depends not only on the breadth of membership coverage but also on the system's ability to maintain financial balance, control costs, ensure accurate subsidy targeting, optimize provider payment mechanisms, and prevent fraud and moral hazard. This article aims to synthesize group materials regarding the structure and implementation challenges of JKN, health insurance utilization, provider payment, digitalization, policy analysis, and financing resilience strategies. A narrative-descriptive review method was employed, analyzing lecture materials, regulations, institutional publications, health economics theories, and expert opinions compiled in the group manuscript. The findings indicate that financial pressures on JKN are linked to increased service utilization, high-cost diseases, contribution compliance, the accurate targeting of subsidized members (PBI), disparities in health facilities, and fraud risks. Perspectives from Hasbullah Thabrany, Nailul Huda, Timbul Siregar, and Febriansyah Budi Pratama underscore the importance of contribution discipline, data transparency, moral hazard control, anti-fraud enforcement, financial protection, and the strengthening of primary care. Financing resilience requires support through strategic purchasing, quality- and efficiency-oriented provider payments, Health Technology Assessment, digitalization, the strengthening of promotive and preventive services, data integration, and accountable governance. Through these strategies, JKN sustainability can be maintained without compromising the principles of mutual assistance (*gotong royong*), equity, and access to necessary healthcare services.
