EFEKTIFITAS PELAYANAN HIPERTENSI DI UPTD PUSKESMAS BANTAR KOTA TASIKMALAYA: STUDI KOHORT RETROPSEKTIF TAHUN 2024

Hipertensi merupakan penyakit tidak menular yang menjadi masalah kesehatan global. Efektivitas pelayanan hipertensi di fasilitas kesehatan tingkat pertama penting untuk meningkatkan pengendalian tekanan darah, kepatuhan pengobatan, dan pencegahan komplikasi pada pasien hipertensi. Tujuan penelitian ini adalah untuk menganalisis efektivitas pelayanan hipertensi di UPTD Puskesmas Bantar Kota Tasikmalaya tahun 2024 berdasarkan ketersediaan obat, kompetensi tenaga kesehatan, dan edukasi kesehatan terhadap outcome klinis. Penelitian ini bersifat kuantitatif dengan desain kohort retrospektif menggunakan data sekunder rekam medis dan dokumen administrasi pasien selama tahun 2024. Analisis data meliputi univariat, bivariat dengan chi-square, dan multivariat menggunakan regresi logistik. Ketersediaan obat berhubungan signifikan dengan pengendalian tekanan darah (p < 0,001; OR = 17,872). Kompetensi tenaga kesehatan berhubungan signifikan dengan kepatuhan pengobatan (p = 0,015; OR = 0,637), sedangkan edukasi kesehatan berhubungan signifikan dengan kejadian komplikasi (p < 0,001). Variabel lainnya tidak menunjukkan hubungan signifikan secara langsung terhadap outcome klinis utama. Analisis multivariat mengonfirmasi ketersediaan obat sebagai prediktor pengendalian tekanan darah, kompetensi tenaga kesehatan terhadap kepatuhan, dan edukasi kesehatan terhadap pencegahan komplikasi.
Hypertension is a noncommunicable disease that poses a global health problem and is known as a silent killer. The effectiveness of hypertension services at primary healthcare facilities is essential to improve blood pressure control, medication adherence, and complication prevention among hypertensive patients. To analyze the effectiveness of hypertension services at UPTD Bantar Public Health Center, Tasikmalaya City, in 2024 based on medication availability, healthcare worker competence, and health education in relation to clinical outcomes. This quantitative study used a retrospective cohort design with secondary data from medical records and patient administrative documents collected during 2024. The sample consisted of 654 hypertensive patients determined using the Lemeshow formula. Data were analyzed using univariate analysis, bivariate analysis with the chi-square test, and multivariate analysis using logistic regression. Medication availability was significantly associated with blood pressure control (p < 0.001; OR = 17.872). Healthcare worker competence was significantly associated with medication adherence (p = 0.015; OR = 0.637), while health education was significantly associated with the occurrence of complications (p < 0.001). Other variables showed no significant direct associations with the main clinical outcomes. Multivariate analysis confirmed medication availability as a predictor of blood pressure control, healthcare worker competence of adherence, and health education of complication prevention.
