POSISI RECOVERY SEBAGAI INTERVENSI KEPERAWATAN PADA ANAK DENGAN KEJANG DEMAM KOMPLEKS: NARRATIVE LITERATURE REVIEW

Abstrak

Latar Belakang: Kejang demam kompleks merupakan kegawatdaruratan neurologis pada anak yang dapat menyebabkan gangguan kesadaran, penurunan refleks proteksi jalan napas, serta meningkatkan risiko aspirasi dan gangguan respirasi pada fase postiktal. Recovery position (posisi lateral) direkomendasikan untuk mempertahankan patensi jalan napas. Tujuan: Menganalisis efektivitas recovery position sebagai intervensi keperawatan pada anak dengan kejang demam kompleks. Metode: Penelitian ini menggunakan metode Narrative Literature Review (NLR). Pencarian literatur dilakukan pada database PubMed dan ScienceDirect. Sebanyak 542 artikel teridentifikasi pada tahap awal pencarian dan setelah proses seleksi diperoleh 5 artikel penelitian primer yang memenuhi kriteria inklusi. Hasil: Seluruh studi menunjukkan bahwa fase postiktal merupakan periode kritis yang rentan terhadap gangguan respirasi. Posisi lateral lebih aman dibandingkan posisi prone dalam mempertahankan jalan napas. Posisi lateral hanya berlanjut menjadi prone pada sekitar 2–3% kasus dan berhubungan dengan risiko gangguan respirasi yang lebih rendah (p<0,05), sedangkan posisi prone lebih sering dikaitkan dengan airway compromise (p<0,05). Gangguan pernapasan seperti apnea dan hipoksemia juga berhubungan signifikan dengan perubahan postur selama dan setelah kejang (p<0,001). Kesimpulan: Recovery position (posisi lateral) merupakan intervensi keperawatan berbasis bukti yang efektif dalam menjaga patensi jalan napas dan menurunkan risiko gangguan respirasi pada fase postiktal, sehingga direkomendasikan dalam penatalaksanaan awal kejang demam kompleks pada anak.

Background: Complex febrile seizures are a neurological emergency in children that can cause impaired consciousness, reduced airway protective reflexes, and increase the risk of aspiration and respiratory problems in the postictal phase. The recovery position (lateral position) is recommended to maintain airway patency. Objective: To analyze the effectiveness of the recovery position as a nursing intervention in children with complex febrile seizures. Method: This study used the Narrative Literature Review (NLR) method. Literature searches were conducted in the PubMed and ScienceDirect databases. A total of 542 articles were identified in the initial search, and after the selection process, 5 primary research articles met the inclusion criteria. Results: All studies showed that the postictal phase is a critical period prone to respiratory disturbances. The lateral position is safer than the prone position for maintaining the airway. The lateral position only turned into the prone position in about 2–3% of cases and was associated with a lower risk of respiratory problems (p<0.05), whereas the prone position was more often linked to airway compromise (p<0.05). Breathing issues like apnea and hypoxemia were also significantly associated with positional changes during and after seizures (p<0.001).Conclusion: The recovery position (lateral position) is an evidence-based nursing intervention that is effective in maintaining airway patency and reducing the risk of respiratory problems in the postictal phase, so it is recommended for the initial management of complex febrile seizures in children.
