Hypertension is a health problem that can lead to serious complications when left uncontrolled. One nonpharmacological intervention that can help lower blood pressure is Epsom salt Foot soak therapy, a foot bath therapy using warm water mixed with Epsom salt. This study aimed to analyze the effect of Epsom salt Foot soak therapy on blood pressure reduction in hypertensive patients in Banyuanyar Kidul Village. This study used a pre-experimental design with a one group pretest posttest approach. The population consisted of 29 hypertensive patients, selected through total sampling. The intervention was carried out for two weeks with a frequency of twice a week. Blood pressure was measured before and after the therapy using a digital sphygmomanometer, then analyzed using the Shapiro Wilk test and Paired Sample T Test. This study obtained ethical approval Number 473/KEPK-UNHASA/VI/2026. The results showed that the mean systolic blood pressure before therapy was 156.97 mmHg and the mean diastolic blood pressure was 98.14 mmHg. After therapy, the mean systolic blood pressure decreased to 139.93 mmHg and the mean diastolic blood pressure decreased to 90.97 mmHg. The Paired Sample T Test showed a p value of 0.000 (p < 0.05), indicating that Epsom salt Foot soak therapy has an effect on blood pressure reduction in hypertensive patients. The high blood pressure before the intervention indicates the presence of risk factors that had not yet been controlled. The decrease in blood pressure after therapy occurred due to the temporary vasodilation effect of warm water, and the relaxing effect of magnesium ions from Epsom salt on vascular smooth muscle after transdermal absorption, making the effect more physiologically targeted compared to a plain warm water foot soak without magnesium content. This therapy can serve as a safe, inexpensive, and easily applicable nonpharmacological alternative to help control blood pressure.
